Timber handrail square dressed with a 3mm pencil round or, for a softer look, a 9mm pencil round on the top and 6mm on the bottom can be run to order in small quantities.  Profiles that require cutters (such as the 47mm diameter) have minimum quantity runs of 50 lm.
